We are currently accepting new patients and can offer same day dermatology appointments. We are a member of most major insurance plans.
Attached is our New Patient Information and Registration Forms. You can save time at our office by printing the forms and bringing them with you for your appointment. Otherwise, please arrive 10 – 15 minutes prior to your appointment time to complete your paperwork. Please bring your insurance card and driver’s license when you come for your appointment. All co-pays, co-insurance, and deductibles are due at time of service. We accept all major credit cards as well as cash and checks.
Please note: If you are making an appointment for a minor child that will drive himself or herself to the office, the child’s guardian will need to be available by telephone for a verbal consent for treatment. A guardian should attend the minor’s first appointment with the office.
